Apply for Meals

Receiving meals from WAMOW is easy, and just requires us to obtain some information from you. 

To complete this process, please fill out and send in the forms below as well as the current menus.  Please fill out the forms to the best of your ability, and do not hesitate to reach out to one of our team members if you have questions or would like to arrange for someone to assist you with the paperwork in your home or over the phone. Once an application is submitted, we will reach out to get delivery details and confirm information you have submitted.  We typically begin service within one or two business days of receiving the completed menus and forms.

Completed hard copy forms may be returned to us via mail to the following address: P.O. Box 453 Wolfeboro, NH 03894, may be emailed to us info@wamow.org, or you can call  (603.525.2043) to arrange for your paperwork to be picked up from your home.
