Receiving meals from WAMOW is easy, and just requires us to obtain some information from you.
To complete this process online, complete the three online forms linked below: Participant Intake Form; Emergency Contact Form; and Menu Selections along with the Recurring Payment Authorization Form and Billing Agreement. The latter two do need to be filled out in hard copy format and returned to us. Please fill out the forms to the best of your ability, and do not hesitate to reach out to one of our team members if you have questions or would like to arrange for someone to assist you with the paperwork in your home or over the phone. Once an application is submitted, we will reach out to confirm a date for initial meal delivery. We typically begin service within one or two business days of receiving the completed menus and forms.
If you prefer to complete hard copy application forms we have provided PDF versions of each form below which you may print from your home computer. Alternatively, you may contact our office by phone (603.525.2043) or email (info@wamow.org) and we will provide you the application in hard copy format. Completed hard copy forms may be returned to us via mail to the following address: P.O.
